(“Company”) and www.parkurda.com (“Platform”) acting in the capacity of Data Controller requires to read and understand Information Notice published in the internet site of the Platform pursuant to our obligation to Inform that arises from Article 10 of the Personal Data Protection Law No. 6698 (“PDPL”) and give your consent with your free will based on the information provided in this Express Consent statement in order to permit the Company to collect, record, process, transfer and store identification data, communication data, personal data, professional experience data, physical space security data, transaction security data, video and audio record data, health data, association membership data and other data for the purposes detailed in the Information Notice and within the limits specified in the applicable legislation and if necessary, transfer of your personal data to competent public organizations and institutions and third persons who are solution partners in the course of business affairs and storage of the data by them.

I hereby represent that I have been informed about Express Consent given in connection with the following matters by the Company in person and based on Information Notice published in the internet site of the Platform, and I have read and understood my rights,

therefore, I hereby agree that the Company and Platform may collect, store, record, process, update, periodically check all kinds of personal data of mine including identification data, communication data, personal data, professional experience data, physical space security data, transaction security data, video and audio record data, health data, association membership data and other data including those that may be used to determine my identify in verbal/ written and/or electronic environment, keep them in databases and if necessary or it is made compulsory in accordance with the applicable legislation, transfer them to third persons and storage of my personal data by such third persons in accordance with the Personal Data Protection Law No. 6698 and Regulation on Processing of Personal Health Data and Ensuring Privacy that has been promulgated in the Official Gazette No. 29863 of 20.10.2016 and this legislation.
